I'm Raj Lakhani, and I built Doctor Notes.
The idea came from my cousin, who's a doctor. He told me he spends an hour or two most nights finishing his notes after his kids are asleep. He isn't unusual. Most doctors do some version of this. The more he talked about it, the more it seemed like a broken way to spend your evenings.
So I built something simple. Hit record, talk to your patient the way you normally would, and get a clean SOAP note back in your own style. You read it, fix anything that's off, and paste it into your chart. That's the whole thing. No clicking through fields, no learning a new system.
I kept it small on purpose. It's for doctors only, it's one fair price, and it doesn't try to do everything. The audio is deleted right after your note is written, and nothing you record is ever used to train AI. Before you record a real patient, we sign a Business Associate Agreement, so the HIPAA side is handled.
One thing I want to be clear about: Doctor Notes is a documentation tool, not a medical device. It doesn't give medical advice. You're always the one who decides what goes in the chart.
If it saves you even thirty minutes a night, it's done its job.
